Promotores(as) and the Patient-Centered Medical Home

Posted by  admin  Aug 8, 2013

From our partner, Migrant Health Promotion:

In 2010, President Obama signed the Patient Protection and Affordable Care Act (PPACA) into law. This new law shifts the focus of healthcare towards lowering cost and increasing quality. As such, the Patient-Centered Medical Home (PCMH) has been gaining a lot of attention recently, as some studies have reported significant cost-savings and improved quality1. The main goal of PCMHs is to emphasize a whole-person orientation and team-based approach to improve healthcare outcomes and decrease costs.  Promotores de Salud can play an important role in the realization of this goal.

To better inform PCMHs and other health organizations about Promotores, Migrant Health Promotion is excited to present the e-learning series, Promotores and the Patient-Centered Medical Home. This three part series defines the basic concepts of Promotores and the Patient-Centered Medical Home, explains the roles and benefits of Promotores supporting a PCMH, and describes how to successfully integrate Promotores into a Patient-Centered Medical Home model.

Session 1: Introduction to Promotores de Salud and the Patient-Centered Medical Home

Session 2: Roles and Benefits of Promotores in a Patient-Centered Medical Home
